Important Tax Information – Available Upon Request
In accordance with the Paperwork Reduction Act, Florida Sheriffs Youth Ranches, Inc./Sheriffs Ranches Enterprises, Inc. will not automatically furnish Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) to employees unless it is requested.
Form 1095-C provides information about health coverage offered by the Florida Sheriffs Youth Ranches. While this form may be used for your personal records, it is not required to file your federal income tax return.
How to Request a Copy
Employees and former employees may request a copy of their Form 1095-C at no cost by using one of the following methods:
Availability Through ADP Workforce Now
If you have access to ADP Workforce Now, your Form 1095-C will also be made available for secure download through your employee self-service portal once it is prepared.
Accessing the form through ADP Workforce Now does not require submitting a separate request.
Employees who do not have access to ADP Workforce Now, or who prefer a paper copy, may request one using the methods listed above.
When You Will Receive the Form
- Requests made on or before January 31 will be fulfilled within 30 days of the request.
- Requests made after January 31 will be fulfilled within 30 days or as soon as reasonably practicable if the form is already prepared.
- Paper copies will be provided unless the employee has previously consented to electronic delivery.
This notice applies to all individuals who were full-time employees or otherwise eligible to receive a Form 1095-C during the applicable tax year.
